For Providers

Medical providers may refer patients to our clinic using the form below or through standard paper or EMR referral, faxed to the Hamilton Birth Control & Sexual Health Clinic at 289-919-2505.

 

Please ensure all patient demographic information and referring medical provider's billing number, phone and fax numbers are included in the referral.

Please note: Referrals are accepted only for patients who either have a valid Ontario health card (or other province), IFH coverage, or UHIP.

 

Our office will contact both the referring provider and the patient with information about the patient's appointment.

For family physicians in a FHO/FHT, a referral ensures our doctors can bill a consult code which will not negate you. In-basket procedure codes will result in negation.
